Step A.
Complete the Police Activities League (PAL) Adult Registration Form .

Step B.
If you are a head coach or team manager, you will also need to be fingerprinted using
the "Live Scan" procedure used by the Santa Clara County Sheriff Department. "Live
Scan" will not be coming to the coaches' or managers' meetings.

Print out the "Live Scan" Reg istration Form Sample. You will need some of the
information on this sample for the next step.
Go to the following website to fill out the online "Live Scan" Registration Form and
to make an appointment to be fingerprinted:

https://sherifflivescan.sccsheriff.org/sfpnew/FingerPrintApplication.jsp .

There is a $10 fee charged by the sheriffs department. WV Slammers will reimburse you
for this fee if you submit your receipt to the league.

"Live Scan" is an independent company hired by the sheriff's department. You may go
directly to this company to be fingerprinted but they charge $20 . WV Slammers will
reimburse you up to $10 so going through the sheriff's department is best.
